Population Health Coordinator
Location: Visalia, CA
Pay: $20.50 per hour (DOE)
Schedule: Monday – Friday | Full-Time
Employment Type: Temp-to-Hire
Position Summary
Key Staffing is currently recruiting for a Population Health Coordinator to join a growing healthcare organization in Visalia, CA. This position serves as a key liaison between healthcare providers, patients, and internal departments to support quality improvement initiatives, preventative care programs, and population health goals.
The ideal candidate will have experience in a healthcare environment, excellent communication skills, and a passion for improving patient outcomes through education, coordination, and outreach.
Essential Responsibilities
- Serve as the primary point of contact between healthcare providers and the Quality & Risk department.
- Support quality improvement initiatives, including HEDIS and Medicare Stars programs.
- Coordinate patient outreach through inbound and outbound phone calls.
- Assist with scheduling appointments and preventative screenings.
- Review medical records and documentation to identify and close gaps in patient care.
- Educate patients and provider office staff on quality initiatives and preventative care measures.
- Review and process provider documentation to support quality compliance.
- Coordinate and participate in community health fairs and outreach events.
- Develop and manage member outreach campaigns to improve healthcare outcomes.
- Monitor, track, and report quality metrics and department performance.
- Identify trends and recommend opportunities for process improvements.
- Maintain accurate records and documentation within electronic medical record (EMR) systems.
- Create and maintain department workflows and administrative processes.
- Collaborate with internal departments and external healthcare partners to achieve organizational goals.
- Provide administrative support and cross-functional assistance as needed.
- Perform additional duties as assigned.
Qualifications
- High School Diploma or GED required.
- Certified Medical Assistant (CMA) preferred, or 3–5 years of healthcare-related experience.
- Previous experience in managed care, IPA, population health, care coordination, or quality improvement is highly preferred.
- CPR Certification preferred.
- Valid California Driver's License with reliable transportation.
- Proficient with Microsoft Office and electronic medical record (EMR) systems.
- Strong organizational skills with exceptional attention to detail.
- Excellent verbal and written communication skills.
- Ability to prioritize multiple projects while meeting deadlines.

Work Environment
- Primarily office-based with occasional travel to provider offices and community outreach events.
